How to register with the practice
In order to register with Rothes Medical Practice, please download and print a registration form and new patient questionnaire from below. Complete all details and hand in to reception with ID for each person registering. Patients are registered with the practice and not an individual G.P (as for all G.P. practices in the UK). Patients consult with the doctor of their choice by making an appointment as explained on the appointment page. If urgent care is required the G.P “on call” at that time will deal with the patient’s care.
