New Registration Forms to join the Practice will not be accepted without valid identification.
Temporary Residents will also need to provide valid identification before being seen by a GP or Nurse.
Forms of Valid Identification include
- Passport
- Driving Licence
- Birth Certificate
- Student ID Card
- Home Office App Reg Card
